RELEASE AND WAIVER OF LIABILITY
I, the undersigned, hereby waive all claims against any and all persons associated with F3 Academy, F3 Sword Academy, F3 Sword Academy LLC, F3 Jiu Jitsu and Straight Blast Gym International (collectively referred to as "F3").
Assumption of Risk
I understand that I am participating in a martial art involving physical body contact. I acknowledge that this training involves risks that could potentially cause serious injury or death, including but not limited to:
• Contact with weapon simulators.
• Choke holds, arm locks, neck cranks, and ankle locks.
• Throws, slams, punches, kicks, elbows, and knees.
• Skin-to-skin contact with other persons and equipment, which may result in contracting skin-borne infections.
Release and Indemnification
I assume full responsibility for all my actions connected to the above organization. I hereby release F3, and all of its agents, employees, and associates from any and all liability, claims, demands, actions, medical bills, and causes of action whatsoever arising out of:
• Any negligent or other acts or omissions.
• Any loss, damage, or injury (including death) sustained by me or my property while participating in this training.
Medical Attestation and Emergency Authorization
I state that I am in good physical condition and know of no reason why I cannot participate in this training. In the event of an emergency:
• I authorize licensed medical personnel to perform any accepted medical assistance deemed necessary.
• I agree to bear the entire expense of any such treatment.
Media and Correspondence Consent
As additional consideration for training at F3, I agree to the following:
• Marketing: My attendance or performance at the gym or events may be photographed, filmed, or taped. I authorize the use of my image for marketing purposes and waive any compensation, even if I discontinue my training.
• Communication: I acknowledge that I would like to receive correspondence from the gym via telephone, mail, and email.
Rules, Conduct, and Membership
I agree to abide by all rules, terms, and conditions of the academy.
• F3 Rules: I have gone over and agree to honor the F3 rules and sparring rules.
• Disciplinary Action: I understand that disobeying rules may result in suspension or expulsion at the instructor’s discretion.
• Refunds/Contracts: In the event of expulsion, I am not entitled to a refund and must pay the remainder of my contract if one has been signed.
Acknowledgement of Agreement
By signing below, I represent that I have read this release, understand it, and sign it voluntarily. I am at least 18 years of age and of sound mind. If the participant is under 18, a parent or legal guardian must sign and agree to all terms.
